Update

As a work around for MFA failures via VoIP phones OIT recommends that users set up MFA on their mobile device:

iOS: https://oit.duke.edu/help/articles/mfa-duo-mobile-setup-ios-device
Android: https://oit.duke.edu/help/articles/mfa-set-duo-android

NOTE: MFA via a mobile device is the preferred method to authenticate.

Another work around is to generate temporary passcodes:

OIT has confirmed a service issue with multi-factor authentication via VoIP phones. OIT staff are in contact with the vendor but there is no estimate of when service will be restored.
